Cessation Worksheets is published by Sunatlohcs Inspirations, based in the Netherlands. Because it operates from the European Union, the General Data Protection Regulation applies to every visitor, wherever you live.

What is collected
If you download a free sheet. Your email address, and nothing else. It is used to send you the sheet and, if you stay subscribed, occasional articles. Every message carries an unsubscribe link that works in one click. Unsubscribing removes your address.
If you email me. Your address and whatever you write. Kept as long as needed to answer, then deleted.
If you only read. The hosting provider keeps standard server logs, including IP addresses, for security and troubleshooting. This site sets no advertising or tracking cookies and runs no analytics that follow you across websites.
No health information is collected anywhere on this site. The worksheets are filled in by your client on their own device or on paper. Nothing typed into a worksheet is sent here, seen here, or stored here.
Who else handles your data
Three companies process data on my behalf, each under a data processing agreement: the hosting provider that runs this site, the email service that sends the mailing list, and — if you buy something — Etsy, which handles the sale under its own privacy policy. I never see your payment details.
Nothing is sold, rented or shared with anyone else.
Your rights
You can ask what is held about you, ask for it to be corrected, or ask for it to be deleted. Email the address above and it is done, normally within a few days and at no cost. If you are unhappy with how a request is handled you can complain to the Dutch Data Protection Authority, the Autoriteit Persoonsgegevens.

What you are buying
A digital download. Nothing is shipped. Files are delivered through Etsy once payment is confirmed.
The practice license
One practitioner, unlimited clients, worldwide, for as long as you practice. You may print without limit for your own clients, use the pages one-to-one and in groups you run yourself, send a blank copy to an individual client by email or through a secure patient portal, and keep a filled copy in a client file where your recordkeeping rules require it.
You may not resell, share or redistribute the files, and the license is not transferable to a colleague — each practitioner needs their own. The full terms come as a separate page inside every download.

Refunds
Digital downloads cannot be returned once delivered, and the right of withdrawal does not apply to them. If a file is damaged, will not open, or is not what the listing described, write to me and it is put right.
What these worksheets are, and are not
They are a support tool for practitioners. They are not medical advice, they do not diagnose anything, and they are not a substitute for the clinical judgment of the practitioner using them. Deciding whether a page suits a particular client is your professional call, not mine.
Every page is reviewed against guidance from Smokefree.gov and MedlinePlus. That means the content is consistent with current patient guidance; it does not mean either organisation endorses these worksheets.
